Notes on Starting Again After a Setback

Across every walk of life, caring for health resembles maintaining anything that will be used for a long stretch of the day. The work is unremarkable, repetitive, and mostly invisible until it is neglected. Nobody notices a roof that does not leak.

Health literacy is not knowing more facts. It is knowing which facts would change a decision, and how confident one is entitled to be.

Knowing one's own normal makes deviations legible.

A few habits of interpretation help. Ask what population a claim applies to; a result from twenty athletes may not generalise. Ask what the comparison is; something that outperforms doing nothing may still be worse than the obvious alternative. Ask about the size of an effect, not just its existence, because a statistically significant improvement can be practically irrelevant. Notice when a relative risk is quoted without an absolute one, since doubling a very small risk leaves a very small risk.

What is difficult is not knowing these things but arranging a life in which they occur reliably, under conditions that are frequently hostile — a job that consumes the hours, a city that discourages walking, an environment engineered to capture focus, a culture that treats exhaustion as evidence of seriousness.

The reasonable defaults have been stable for a long stretch of the day and are boring: mostly plants, adequate protein, regular movement including some resistance, sufficient sleep, minimal smoking, moderate or no alcohol, some human contact, appropriate screening. Almost everything else being marketed is optimisation at the margins, and margins matter only after the centre is in order.

And keep the purpose in view. Health is not a score, an appearance, or a moral status. It is the capacity to do the things that make a life worth having, retained for as long as circumstances allow.
